Your health plan or program sponsor is working with ExamOne, a Quest Diagnostics company, to provide you a health screening. Depending on your program, you may have an in-home visit to collect lab specimens, or an appointment at one of our 2,200 patient service centers across the country. In some cases, a licensed nurse practitioner will also visit you in your home for a follow-up assessment.
Preparing for your appointment
It starts with a phone conversation. An ExamOne representative will contact you to set up an appointment and ask you a few questions about your health history. This is your chance to learn about the process, ask questions and address concerns.
During your appointment
Next is a quick assessment, which can be done in the convenience of your own home or at one of our easily accessible office locations. Our trained technicians will perform a quick screening that includes height, weight, blood pressure, pulse and specimen collection.
After your appointment
After your appointment, you will receive a copy of your lab results in an easy-to-read format.
If your health plan or program sponsor is participating in our in-home assessment program, a licensed nurse practitioner will make a follow up visit to your home after the laboratory test results are complete. The nurse practitioner will spend about 45 minutes discussing your results and learning more about your health. The nurse practitioner will also discuss customized preventive screening services targeted to your history.
If your program does not include the follow-up visit from the nurse practitioner, you will receive a copy of your lab results in the mail. For all programs, lab results will also be mailed to your primary care physician on file to assist your doctor in future care planning.
